Skilled Nursing

 Skilled nursing services are provided by licensed professionals to help patients manage medical needs safely at home. This may include clinical assessments, vital sign monitoring, disease management support, post-hospital follow-up, medication education, and coordination with the physician to promote recovery and reduce the risk of complications 

 

Diabetic Care

 Diabetic care focuses on helping patients safely manage diabetes in the home setting. Services may include blood sugar monitoring support, education on diabetes management, observation for signs of complications, foot care guidance, medication and insulin routine reinforcement, and coordination with the physician as part of the overall care plan. 

Wound Care

 Wound care services are designed to support healing and reduce the risk of infection or further complications. Care may include wound assessment, dressing changes, monitoring of healing progress, skin integrity support, patient and caregiver education, and communication with the physician regarding the patient’s condition. 

 

Infusion Therapy

 Infusion therapy provides clinical support for patients who require medications or treatments administered through infusion in the home. Services are coordinated according to the physician’s orders and may include monitoring the patient’s response to treatment, observation for adverse reactions, care coordination, and patient education for safe treatment support. 

 

Private Duty Nursing

 Private duty nursing offers more continuous one-on-one nursing support for patients with ongoing medical needs. This service is appropriate for individuals who require close monitoring, skilled assistance, and personalized care in the home environment over an extended period of time. 

 

Occupational Therapy

 Occupational therapy helps patients improve their ability to perform everyday tasks such as bathing, dressing, grooming, and other daily routines. Services focus on building functional independence, improving safety, and adapting activities or home environments to better support the patient’s daily life. 

Speech-Language Pathology

 Speech-language pathology services support patients with speech, communication, cognitive, or swallowing difficulties. Treatment may focus on improving communication abilities, strengthening swallowing safety, and helping patients function more effectively in daily life. 

Medication Management

 Medication management helps patients follow prescribed treatment plans more safely and consistently. This may include medication education, schedule reinforcement, observation for adherence concerns, support in understanding instructions, and communication with the care team regarding medication-related issues. 

Laboratory Services

 Laboratory services coordination helps ensure that ordered lab-related care needs are supported efficiently as part of the patient’s treatment plan. This may include assistance with care coordination, communication with providers, and support related to ongoing monitoring requirements. 

Personal Care & Activities of Daily Living

 Personal care services support patients who need assistance with essential day-to-day activities such as bathing, grooming, dressing, mobility, and other routine care needs. These services are designed to promote comfort, dignity, safety, and quality of life in the home setting. 

Nutritional Guidance

 Nutritional guidance supports patients whose recovery, health condition, or treatment plan may benefit from better nutrition practices. Education and support may help patients and families understand dietary recommendations and reinforce nutrition as an important part of the overall plan of care.
